Provide survey URL (EXPOSUREAPP-4858) (#2307)

* Create SurveyUrlProvider.kt and SurveyHighRiskUrlProviderTest.kt

* Call SurveyUrlProvider in Surveys.kt

* Adapt SurveyUrlProvider.kt so that now the otp has to be passed directly

* Add @Reusable annotation to SurveyUrlProvider.kt
